Fatal Collision in Leslie County

Date of News Release: 08/07/2013

(Hazard, KY) -- On Tuesday, August 6, 2013 at approximately 1709 hours, Post 13 Hazard was notified of a two vehicle injury collision with one possible fatality, located at approximately 5100 South US Highway 421 in the Helton Community of Leslie County.

Units arrived on scene at approximately 1748 hours and conducted a preliminary investigation. It was determined Unit 1, a 1997 Ford Explorer, was traveling south on this roadway and was operated by Tracey L. Collett, age 39 of Warbranch, KY. Unit 2, a 1999 Ford F-150 was traveling north on this roadway towing an eighteen foot Ranger bass boat and was operated by Wilburn G. Pennington, age 49 of Hyden, KY. No additional occupants were located in Unit 1 and one passenger was identified in Unit 2, Glen Caldwell, age 45 of Wooton, KY. Roadway evidence, the position of these vehicle at final rest and a witness statement revealed the operator of Unit 1 veered into the north bound lane, while traveling south, and entered into the direct path of Unit 2 causing a head on collision. The operator of Unit 1, Tracey Collett, was pronounced deceased on scene by the Leslie County Coroner at approximately 1720 hours. The operator of Unit 2, Wilburn Pennington, and the passenger of Unit 2, Glen Caldwell, was transported to Holston Valley Medical Center in Kingsport, TN. Wilburn Pennington suffered life threatening injuries, but is currently in stable condition and Glen Caldwell suffered non-life threatening injuries. Trooper Justin Oliver U/407 continues the investigation of this collision and Trooper Danny Clark U/ 290 continues Death Investigation 13-13-0479. Units were assisted on scene by Trooper Loren Holiday U/598, Leslie County Sheriff Department and the Leslie County Coroner.
