Fatal Collision on US 41 in Todd County
Date of News Release: 02/18/2009
(Madisonville, KY) - Traci Grant was operating a 1997 Jeep Grand Cherokee southbound on US 41. George Toliver was operating a 1997 Ford F-150 pickup truck northbound on US 41. Carolyn Evans was operating a 1998 Ford Windstar Van northbound on US 41 traveling behind Mr. Toliver. For an undetermined reason, the Jeep Grand Cherokee crossed the centerline and traveled into the northbound lane striking Mr. Toliver's vehicle in a head-on manner. Mr. Toliver's vehicle exited the northbound shoulder of the roadway and traveled into a ditch where it came to rest. Ms. Evans attempted to avoid the collision by steering into the ditch line off the northbound shoulder of the roadway, but struck Mr. Toliver's vehicle in the ditch. The Jeep Grand Cherokee reentered the southbound lane and came to rest on the roadway. Mr. Toliver was pronounced dead at the scene by the Todd County Coroner, Gerald Barnett. Ms. Evans and her three passengers were not injured. A passenger in the Jeep Grand Cherokee, Adam Grant, was pronounced dead at the scene by the Todd County Coroner. Ms. Grant was flown from the scene to the Vanderbilt Medical Center. The other two passengers were transported to the Jennie Stuart Medical Center. Both were later transported to the Vanderbilt Medical Center. The investigation is continuing by the Kentucky State Police.
