Two Vehicle Injury Collision on US 641

Princeton, KY (Caldwell County) - The Kentucky State Police investigated a two-vehicle, injury collision that occurred on US 641, south of Fredonia, on Sunday, May 03, 2015 at 10:15 a.m.

The preliminary investigation revealed that Robert Riley, 72, Fredonia, was operating a 2011 Chrysler 200 northbound on US 641 when he appears to have had a medical emergency and lost control of the vehicle. The vehicle traveled into the southbound lane where it collided with a 2010 Chevrolet Silverado operated by Jeremy Tackett, 35, Sturgis, which was traveling southbound. Mr. Riley's vehicle then struck a ditch on the northbound shoulder causing his vehicle to overturn. Mr. Tackett's vehicle exited the southbound shoulder of the roadway and struck a utility pole.

Mr. Riley was transported via ambulance to the Crittenden County Hospital for treatment. Mr. Tackett, his wife Shelly, 31, and daughter Jaylen, 3, were transported via ambulance to the Caldwell County Hospital for treatment. Mr. Tackett's other passenger, daughter Brooklyn, 7, was not injured during the collision. Everyone involved were wearing seat belts.

Tpr. Ben Sawyer investigated the collision. The Caldwell County Sheriff's Department, Fredonia Fire Department, Princeton Fire Department, and Caldwell County EMS assisted at the scene.
