Two-Vehicle Injury Collision on East Center Street and Park Avenue

Madisonville, KY (Hopkins County) - The Kentucky State Police investigated a two-vehicle, injury collision that occurred at the intersection of Park Avenue and East Center Street, on Wednesday, February 12, 2014, at 6:26 p.m.

The preliminary investigation revealed that Audriana Coyazo, 17 of Madisonville, KY, was operating a 2002 Dodge Intrepid westbound on East Center Street. Laura Burton, 41 of Madisonville, KY, was operating a 2008 Dodge Caravan eastbound on East Center Street. Ms. Coyazo was attempting to make a left turn on Park Avenue and failed to yield to oncoming traffic, turning in front of Ms. Burton. Ms. Burton's vehicle struck the passenger side of Ms. Coyazo's vehicle.

Marissa Coyazo, 15 of Madisonville, KY, a passenger of Audriana Coyazo, was transported via ambulance to Baptist Health Madisonville for treatment. She was not wearing a seatbelt.

Audriana Coyazo and her other passenger, Lindsey Sutton, 16 of Madisonville, KY, and Laura Burton and her passenger, Ezra Burton, 9 of Madisonville, KY, were not injured and were all wearing their seatbelts.

Trooper Jonathan Murphy investigated the collision. Trooper Nathan Craft, Madisonville Police Department, and Med Center EMS assisted at the scene.
