Five Taken to the Hospital After Injury Collision

Madisonville, KY. (Hopkins County) The Kentucky State Police investigated a single- vehicle injury collision that occurred on Grapevine Road, approximately two-tenths of a mile south of Madisonville, on Tuesday, April 24, 2012 at approximately 12:55 p.m.

Preliminary investigation revealed that Angel G. Ortiz, age 24 of Madisonville, KY, was operating a 1997 Jeep Grand Cherokee northbound on Grapevine Road. Mr. Ortiz advised that he swerved to the right to avoid an approaching vehicle that was in his lane.

Mr. Ortiz's vehicle exited the right side of the roadway and traveled into a ditch. It continued to travel through the ditch and struck a culvert. The vehicle became airborne and landed on its top.

Mr. Ortiz and his passengers, Christina Ortiz (age 25), Elijah Ortiz (age 1), Gabriel Ortiz (age 2) and Angel Ortiz (age 3) were transported via ambulance to the Regional Medical Center for treatment. None of the occupants were using restraints.

Trooper First Class John Komar investigated the collision. The Grapevine Volunteer Fire Department and Medical Center Ambulance Service assisted at the scene.
