Injury Collision on I-64 in Carter County

Date of News Release: 03/02/2012

(Ashland, Ky.) -- The Kentucky State Police in Ashland is investigating a two vehicle collision that occurred on I-64 Eastbound early this morning (03/02/2012) near the rest area in Carter County, KY. The collision, which occurred around 4:51 am, involved a passenger vehicle and a commercial vehicle. Columbus Hollingsworth, age unknown, of Danville, Virginia was operating a 1978 Ford Van traveling eastbound. Jeffery Couch, age 41, of Nancy, KY was operating a 2008 Kenworth Tanker Trailer also traveling eastbound behind Hollingsworth. Apparently, Hollingsworth attempted to change lanes into the fast lane of I-64 when Couch allegedly changed lanes too causing the vehicles to collide. This caused the van to strike a guardrail. The initial cause of the collision is still under investigation.

Fuel from truck's fuel tank spilled onto I-64 causing the roadway to be shut down for two hours. At this time, one lane of I-64 eastbound is open to traffic. Hollingsworth was transported by Carter EMS to King's Daughters Medical Center for non-life threatening injuries.

Carter County EMS, Grayson Fire Department, and Commercial Vehicle Enforcement assisted at the scene.

The collision is being investigated by Trooper Daniel Cook.
