Triple Fatality on KY 295 in Crittenden County, KY

(Madisonville, KY) - Kentucky State Police Post 1 and Post 2 units responded to the scene of a fatal collision on KY 295, one mile South of Dycusburg, in Crittenden County. The investigation revealed that the driver of the Firebird was northbound and overtaking another vehicle in a no passing zone. The driver did not re-enter the northbound lane in a timely manner, according to witnesses, and struck an oncoming vehicle, driven by Ms. Lefan, head on at the crest of a hill. Ms. Lefan made an attempt to avoid the collision by applying her brakes and steering her vehicle in an evasive manner, but was unable to do so.

After the collision, Ms. Lefan's vehicle came to a final rest partially in the roadway and partially on a culvert. She was flown by helicopter to Deaconess Hospital in Evansville, Indiana, where she later died as a result of her injuries. The Firebird came to a final rest off of the roadway and caught fire as a result of the collision. Both occupants were pronounced deceased at the scene by the Crittenden County Coroner. The identities of those occupants are known, however, they are being withheld at this time pending further investigation.

It is believed that none of the victims were wearing their seatbelts at the time of the collision. The collision is still under investigation and no further information is available at this time.
